Research work/Study for Nobel Prize in Physiology or Medicine
For ADULT patients:
(a) 'Silence' HLA gene on donor organ, then proceed with organ transplantation: Transfect macrophage, L929, or HEK293 cells with siRNA (silencing RNA) mixture consisting of HLA-A, HLA-B, HLA-C, HLA-DPA1, HLA-DPB1, HLA-DQA1, HLA-DQB1, HLA-DRA, HLA-DRB1 (to be developed)(1,2,3,4,5,6,7). Grow cells till they reach 90% confluence, isolate siRNA from cells, add siRNA mix to 'UW' solution (8,9,10) at concentration of 100 micrograms/ml. Place donor organ in this solution @4 degrees celsius for 48 hours (8). Proceed to organ transplantation, step (b).Note: Due to the current price of siRNA, option (a) is therapeutically VERY EXPENSIVE and NOT FEASIBLE unless the patient is VERY RICH
(b) If step (a) fails/not economically feasible, proceed to Thymectomy first; then perform organ transplantation
For MINOR patients (kidney, liver):
(c) Organ transplantation
(d) Sirolimus monotherapy (11,12)
